TRANSITIONAL SERVICES

CT certified and specially trained Personal Care Assistants, Homemakers, Companions, Job Coaching, and Case Managers are available to assist clients transitioning from an institutional setting, employment position , or who have already made the transition to living independently but are looking for additional services.

Under the Mental Health Waiver, Money Follows The Person, or the Elder Care Waiver programs and DSS, Moving Forward Consulting staff will work with your team of service providers, (or help you gather a team), to create a Person Centered Plan to guide us in facilitating your recovery and adjusting to community settings while living independently.  As we follow the Plan, we will provide our clients, and families, with compassionate insightful interaction and connect them with helpful community outlets.  

We can work with you even if you are not on a waiver.  (Moving Forward Consulting Case managers can assist you in applying for a waiver.)  Our first one hour consult is free. 

Elder Care

Moving Forward Consulting case managers and caregivers are caring, kind, dependable and personable individuals who desire to provide non-medical, in-home care to the elderly. Our caregivers often become like a surrogate family member helping to meet the needs of our clientsWe help people stay in their homes for as long as possible by providing things such as community advocacy, companionship, meal preparation, medication reminders, transportation to non-medical events, light housekeeping and errands. Duties also include personal care such as bathing, toileting and grooming.

Employment Placement

We will provide an individual with one-on-one skills training, planning, and advocacy services for all aspects of the employment process in relation to their job. We include other service professionals and resources in the service delivery process as needed to enhance effective services. Duties include but are not limited to: 

  • assessing the strengths and needs of individuals in relation to their performance and job setting;
  • designing strategies appropriate to assist the individual to make progress in their placement in terms of productivity, interpersonal skills, quality of work and behaviors;
  • providing follow-up and job retention counseling services;
  • visiting potential job sites;
  • attending interviews, etc.; 
  • providing employers with crisis and technical assistance;
  • providing needed advocacy in discussion with benefits retention.
  • providing intervention and support to maintain employment;
  • assisting with job development activities;


 

Services we offer include:

   Case Management:

         Duties include but are not limited to:

  • connecting clients to useful community programs (housing, food, medical, social, etc.);
  • completing program applications;
  • hiring/firing/supervising staff;
  • coordinating finances
  • aiding individuals in obtaining needed support services such as clinical treatment, transportation, residential, etc.;
  • coordinating efforts with families, schools, etc.;

   Homemakers/Companions:

         Duties include but are not limited to:

  • providing caring companionship & conversational;
  • providing or arranging transportation to errands, appointments, etc;
  • meal preparation and planning;
  • light housekeeping;
  • finding/assist working on a hobby/craft;
  • general assistance.

  Personal Care Assistance:

        Duties include but are not limited to:

  • all Homemaker/Companion duties;
  • heavy housekeeping (changing bedding, washing/vacumming floors, sanitizing bathrooms, removing trash, laundry, cleaning appliances, dusting);
  • assistance with bathing, dressing & grooming;
  • Assistance with administration of medications.

   Transportation:

        Duties include but are not limited to:

  • assisting clients in getting to and from doctor appointments, grocery shopping, errands, etc.;
  • assisting in contacting or arranging community transportation;
  • providing safe and insured transportation personally
  • assisting the client with acquiring finances for public transportation;

   Education:

        Duties include but are not limited to:

  • life skills retraining;
  • conservator training;
  • supervisory training;
  • computer training;
  • academic tutoring;
